Building an Application Form

Design the questions applicants answer.

The form builder is the Application Form step of a program's Settings tab.

What is already there

Three fields exist on every form and are marked STATIC. You cannot remove or reorder them:

  • #1 Name — Text, required.
  • #2 Email Address — Email, required.
  • #3 Industries — Select, required.

Starting from an existing form

Use Import From Program — “Use previous application form as a template?” — to copy the questions from another program rather than rebuilding them.

Adding your own questions

Questions live inside sections, so create a section first.

  1. Turn on Collect Applications, described as “Enable to require applicants to submit a form.”
  2. Select Add New Section and give it a Section Title, plus an optional Description. Save.
  3. Inside the section, select Add Question.
  4. Choose a field type, then fill in the Question Label. Placeholder Text and Help Text are optional, and Required is a Yes or No choice.
  5. Save the question, and repeat. Add further sections as needed.

Sections can be reordered by their drag handle and each has its own actions menu; individual questions have an actions menu too.

Conditional logic

A question can be shown only when an earlier answer matches. The dialog notes that conditional logic is only available if there are previous radio or single select questions in the form, so put the branching question before the questions that depend on it.

Checking your work

Select Preview Form to see the form as an applicant will. Next stays disabled until you either import a template or add at least one question.

Please note: The form locks once the first application is submitted, so finalize your questions before opening the window.
